本人聲明身體狀況良好，無醫師建議不得從事激烈運動之疾病或傷害。

I declare that I am in good health and have no medical restriction or advice against participating in strenuous physical activities.

本人了解激流獨木舟及相關水域活動具有一定危險性，即使已採取合理安全措施，仍可能發生意外事故。

本人已充分了解並願意承擔包括但不限於以下風險：

• 船艇翻覆、落水及游泳脫離船艇
• 撞擊岩石、河床、樹木、橋墩或其他天然及人工障礙物
• 被倒木（Strainer）、漩渦、回流、水壓區或其他河川地形困住
• 裝備損壞、遺失或失效
• 體力透支、抽筋、脫水、中暑或失溫
• 天候、水位、水流變化造成之危險
• 野生動物、昆蟲叮咬或其他自然環境風險
• 偏遠地區救援及醫療延誤
• 身體受傷、永久失能或死亡

I understand that river kayaking is an adventure activity involving inherent risks. Despite reasonable safety precautions, accidents may still occur.
I acknowledge and voluntarily assume all known and unknown risks associated with participation in the course.
本人係基於自由意志決定參與本課程，並願意承擔活動固有風險。

本人同意：

• 遵守教練及工作人員之安全指示
• 全程正確穿戴規定之安全裝備
• 不於活動期間飲酒或使用影響判斷能力之藥物
• 不從事超出教學範圍或教練許可之行為

若本人拒絕遵守安全規範，教練有權立即終止本人參與課程，且不予退費。

I agree to follow all safety instructions, wear proper gear, and refrain from alcohol or drugs that impair judgment. If I fail to comply, the instructor has the right to terminate my participation immediately without refund.

如於活動期間發生意外事故、受傷或突發疾病，而本人無法即時自行決定時，本人授權教練、工作人員或相關救援單位聯絡緊急聯絡人、協助送醫並接受必要之緊急醫療處置。本人了解並同意因此產生之醫療、救護、救援、後送、住院及相關費用由本人或本人之保險負擔。

In the event of an emergency, I authorize instructors or staff to obtain necessary medical treatment on my behalf, and agree to bear all rescue and medical expenses.

主辦單位將依法或依課程規劃為參加者投保相關保險（如水域活動意外保險或其他適用保險）。本人了解：保險內容依實際保單條款為準、非所有損失均屬理賠範圍、超出保險理賠範圍之損失，應依相關法令及實際責任歸屬處理。

The organizer may provide accident insurance coverage for participants. Coverage and compensation shall be subject to the terms and conditions of the applicable insurance policy.

本人同意主辦單位於課程期間拍攝之照片、影片或其他影像紀錄，得用於教學紀錄、ACA 認證申請、活動成果紀錄、網站、社群媒體及招生宣傳。使用時不另行通知，亦不另支付報酬。

I grant permission for photographs and video recordings taken during the course to be used for educational, certification, documentation, and promotional purposes without compensation.

本人了解：課程結束後是否取得 ACA 認證資格，將由授課教練依 ACA 標準進行評估。參與課程並不保證取得 ACA 證書或認證資格。

Successful participation in the course does not guarantee ACA certification. Certification decisions are made solely according to ACA standards and instructor evaluation.
